The Community Service Book (CSB) Online Edition is a group of valuable coupons which is accessable via the Internet. The sale of the Online Book will generate much needed funds and can be operated without having to leave your desk. Sale setup is easy and begins when you, the program leader, submit this form. CSB will confirm your user name, your password and provide you with a unique link. You communicate the unique link to your members (via email, print material, link on your web site, etc...). Sales are generated when family and friends click the link and purchase an Online Book. Proceeds to your organization are 50% of the purchase price. Proceeds from the sales will be accumulated by CSB and distributed monthly via check to the official named on this application. Reports are available online so you can monitor sale progress.
